Squeeze In
All patients must have a scheduled appointment. We will make every effort to schedule an appointment on the same day of the call for patients experiencing urgent health matter such as pregnancy complications, sick children, acute infection, etc. If you squeeze in during another patient's visit, this may be disruptive to the schedule and lead to longer wait time for other patients. Please be considerate of this.

Late for Appointments
Patients are expected to come to their appointment on time and we will make every effort to see you in a timely fashion. If you are running late, please let our office know as there are other patients who also have appointments, and time will be disruptive which will cause delays.

Patient Assessment
To thoroughly assess concerns and to better accommodate scheduling, we request that patients focus on only one or two health concern per visit. If you feel you may require extra time for a complicated issue or counseling, please inform our staff when booking an appointment so appropriate time will be allotted.

Cancellation & No Show
If you are unable to attend your appointment, please cancel at least 24 hours in advance. Failure to do so disrupts our schedule and a fee will be charged. Please make sure to attend specialist appointments as any re-referral will need to be discussed with the doctor.

Missed Phone Consults
Missing your phone consult will result in a fee. We encourage you to make sure you are available. If you miss the doctor’s call, you will have to rebook your appointment. Please note that the doctors may call you from an Unkown or No Caller ID number, please be mindful to pick up the call.

Test Results
We encourage patient to routinely book follow-up appointments after any tests have been ordered as we do not disclose any type of results through phone or fax. This is for your own safety and privacy.

Termination
Termination of the physician-patient relationship can be done at any time and can be initiated by either party. If a patient feels that the relationship is not productive, they can seek a new doctor. The physician can also decide if the relationship is not therapeutic, he or she will notify the patient if necessary.

Confidentiality
Our patients will be treated with the highest level of confidentiality from our office. The patient must provide a written, witnessed authorization in order to release medical information to anyone including but not limited to parent, counselor, spouse unless you are with the person at the time of disclosure.

Our Staff & Zero Tolerance for Abuse
Please treat our staff with respect and courtesy as we are trying to give our best service. We value the work done by our staff and feel that both our staff and patients deserve a respectful environment. The physicians have zero tolerance for abuse of our staff as well as other patients in our clinic. Any concerns or feedback about the service you receive can be brought to the attention of your physician.

Health Record Policy
The health information that we are collecting is needed to provide you with diagnostic treatment and care services. It is collected under the authority of section 20(b) of the Health Information Act - directly related to and necessary to carry out an authorized purpose under section 27. The confidentiality of this health information and your privacy are protected by the provisions of the Health Information Act.

Inactive Patient
According to Alberta Medical Association, if you have not been in the clinic for the past 3 years, your file will be considered inactive. Unfortunately, this means that we will not be able to accept you as a patient at our clinic in the future. Our doctors are currently at full capacity, and we want to ensure that we are able to provide timely and effective care to all of our patients.
We understand that finding a new family doctor can be challenging, so we strongly encourage you to schedule a timely follow-up appointment with your family doctor to avoid this situation.
